Your first visit to our office establishes a vital foundation for our relationship with you. We will listen closely to your concerns and conduct a thorough, comprehensive examination. We will take the time to provide you with the personal attention you deserve.
Before any treatment begins, we will discuss with you your dental expectations and your dental health needs. We can then advise you of your options so that you can make an informed choice regarding the best course of treatment for you. We have the utmost respect for our patients and our goal is to provide you with the highest quality care in an atmosphere of mutual trust.
Our dental office is truly a family practice that is based on word of mouth referrals.
What to bring to your first visit:
Please bring in a list of your current medications.
If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them with you to your appointment. If additional x-rays are necessary, they can be taken at our office.
Any dental insurance information would help us to assist you in processing your claim.
IMPORTANT: A parent or guardian must accompany all patients under 18 at their visit.

If possible, please print and complete the Health History form. Doing so will save you time on your first visit.
We also have our Financial Arrangement form available to get you better acquainted with our policy. We accept Cash, Check, Visa, Mastercard and utilize Care Credit for more affordable payment options.

Insurance and Billing
We accept many insurance plans, contact our office to verify acceptance of your plan. We do not participate in Health Management Organizations; however, we will be happy to file your insurance claims for you.. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.
