In order to streamline your first visit, we have our new patient form available for you below and a list of items that you need to bring to the office.

In order to streamline your first visit to our office, download our new patient Health History Form and fax it back to us or bring it with you to your appointment. When you arrive for your appointment, your paperwork will already be filled out and you can go in for services without waiting.

What to bring on your first visit:

• a list of any medications you are currently taking
• if your previous dentist has taken recent x-rays (within 6-months), you may ask that they forward them to our office. If there is not enough time, please pick them up and bring them with you to your appointment. If additional x-rays are necessary, they can be taken at our office.
• Your dental insurance information.
